Treatment of Biliary Infections with Ampicillin.

چکیده

EXPERIMENTAL evaluation of ampicillin has proved that in animals this drug behaves like penicillin G in being rapidly excreted in high concentrations in the bile after oral, intramuscular and intravenous administration (Table I). The ratio of bile to blood (or serum) concentration varies with the species of animal, route of administration, dose, time and technique of sampling. However these results, and the few observations in man, suggest that the administration of ampicillin to patients with inflammatory diseases of the biliary tract and gallbladder should result in biliary levels above the M.I.C. for micro-organisms likely to cause infections of this system (e.g. streptococci, Strept. fecalis, Esch. coli, various salmonelle, etc.). Since the organism responsilble for biliary infection is frequently not isolated, tests to determine the M.I.C. may not be available. Therefore the results of treating biliary infection with ampicillin were judged by clinical observations and indirect laboratory data.

Cefoperazone compared with ampicillin plus tobramycin for severe biliary tract infections.

In a prospective, randomized, multicenter study, the efficacy and safety of cefoperazone and the combination ampicillin-tobramycin as initial therapy for patients with severe acute biliary tract infections were compared. Of 77 patients initially entered in the study, definite severe biliary tract infection was confirmed in 67. Sixty-four patients completed treatment. At the end of treatment, 35...

Multidrug Resistance in Infants and Children

Bacterial infections may cause disease and death. Infants and children are often subject to bacterial infections. Antimicrobials kill bacteria protecting the infected patients andreducing the risk of morbidity and mortality caused by bacteria. The antibiotics may lose their antibacterial activity when they become resistant to a bacteria. The resistance to different antibiotics in a bacteria is ...

A Review of Combination Antimicrobial Therapy for Enterococcus Faecalis Bloodstream Infections and Infective Endocarditis.

Enterococci, one of the most common causes of hospital-associated infections, are responsible for substantial morbidity and mortality. Enterococcus faecalis, the more common and virulent species, cause serious high-inoculum infections, namely infective endocarditis, that are associated with cardiac surgery and mortality rates that remained unchanged for the last 30 years. The best cure for thes...

The value of chemoprophylaxis against Enterococcus species in elective cholecystectomy: a randomized study of cefuroxime vs ampicillin-sulbactam.

HYPOTHESIS Cephalosporins are widely used and considered to be effective as prophylaxis in biliary surgery. Nevertheless, they lack activity against enterococci. We conducted a study to compare the efficacy of ampicillin-sulbactam vs cefuroxime in preventing surgical site infections following elective cholecystectomy. DESIGN A prospective randomized controlled trial. SETTING A major tertiar...

Ampicillin levels in human bile in the presence of biliary tract disease.

Ampicillin levels were measured in the serum and in the bile from both the gall bladder and the common bile duct in patients undergoing surgery for biliary tract diseases. In patients with radiologically non-functioning gall bladders ampicillin was either not present or its concentration was lower than normal. Therapeutic levels were present in the common bile duct of all patients except those ...
